Fourteen months after my hip replacement, my surgeon told me the X-ray was beautiful. Clean lines, good position, a textbook success. And yet there I was, still reaching for the banister on my own staircase, still not feeling strong in that leg.
The joint was perfect. So where was my confidence? That question sent me back into the research, and what I found reorganized how I think about my own recovery. It has a name: sarcopenia.
What sarcopenia actually is
Sarcopenia comes from the Latin roots sarco, meaning flesh, and penia, meaning poverty. Poverty of the flesh. It is the age-related loss of muscle mass and, most importantly, muscle strength.
Here is the timeline nobody hands you: our muscles hit peak performance around age 25. Strength declines about 1 to 2 percent per year between 50 and 60, and after our sixties that accelerates to roughly 3 percent per year. Estimates of how many older adults have sarcopenia range from 1 percent to 29 percent, with some estimates far higher depending on lifestyle. That range is wide because researchers define it differently, but the direction is not in dispute.
And the scale in your bathroom will not tell you any of this. Your total weight says nothing about how much of you is lean tissue and how much is fat. If we want to protect our muscles, we deserve to know the difference.
The loop: a bad joint costs you muscle
Here is what got personal for me. I spent four years quietly protecting a joint that hurt, and I never understood what that protection was doing to my muscles.
Researchers who study knee osteoarthritis and sarcopenia together describe a relationship that runs in both directions. Sarcopenia can speed the progression of arthritis, and arthritis worsens sarcopenia, because a painful joint makes you move less. Less load on the muscle means less muscle. Less muscle means less support for the joint. It is a loop, and pain keeps it spinning.

Why walking is not enough
I walk every day, and I always will. But walking does not send the signal that tells muscle to rebuild. Resistance does.
A systematic review found that resistance training on its own improves muscle mass and strength in older adults. The effects are small to moderate, which is not disappointing, it is realistic. Even holding steady is a win when 3 percent annual decline is the default. The evidence points to two to five sessions per week at a meaningful effort, done safely: good form, low weights, more repetitions, then progress slowly.
Protein matters too. The common guidance for older adults is at least 1 to 1.2 grams per kilogram of body weight per day, and I will be honest, I find that hard to hit. One caveat I take seriously: if you have kidney issues, talk to your doctor before raising your protein substantially. Vitamin D and good sleep round out the picture.
The 30-second chair stand test
This is the part I want you to actually do. It is a validated fall-risk screening you can run in half a minute. Put a sturdy chair (about 17 inches high) against a wall. Sit in the middle, back straight, feet shoulder-width apart, arms crossed at your wrists against your chest. For 30 seconds, stand fully and sit back down as many times as you can.
Reference ranges for women
- Ages 60 to 64: about 12 to 17 stands
- Ages 65 to 69: about 11 to 16 stands
- Ages 70 to 79: about 10 to 15 stands
- Ages 80 to 84: about 9 to 14 stands
- Ages 85 to 89: about 8 to 13 stands
If your number came in low, hear this: it is not a verdict. It is a baseline. Write the number down with today's date, work on strength three days a week, and retest in four to eight weeks. Mine was low too. We start where we are.
